Switching medicines.

Your doctor sets your starting dose from your history.

Two doctor.uk treatment boxes side by side.

No direct dose conversion

The medicines work differently and are dosed on different scales. No trial has matched their doses step by step, so there’s no approved conversion chart.

Medicine How it is taken Licensed dose range Available as a tailored plan
Mounjaro (tirzepatide) Weekly injection 2.5mg to 15mg Yes
Wegovy (semaglutide) injection Weekly injection 0.25mg to 7.2mg No
Wegovy (semaglutide) tablets Daily tablet 1.5mg to 25mg No — a tablet cannot be dose-adjusted the way a pen can

What your doctor considers

Your doctor also checks your other conditions, medicines, pregnancy plans and any upcoming surgery.

How you tolerated it

Often more useful than the dose you reached.

Why you’re switching

Side effects, cost, supply, plateau or preference.

The direction

A stronger medicine, a milder one, or injection to tablet.

How long you held your dose

A year at a dose is different from four weeks.

Losing or maintaining

Switching mid-loss differs from switching to hold.

The gap

These medicines clear slowly, so timing matters.

Injection to tablet

In the ATTAIN-MAINTAIN trial, 376 people at a plateau on semaglutide or tirzepatide switched to daily orforglipron or placebo. At 24 weeks, those switching from semaglutide averaged −0.1kg and those from tirzepatide +2.6kg, against about +9kg on placebo.

What to bring

Tell your doctor

  • What you take, and the dose
  • How long you’ve been on it
  • How it affected you at first, and now
  • Your last dose date, and your longest gap
  • Anything taken from other sources
  • What you currently pay

After a switch, expect a short adjustment period, much like starting for the first time.
